Tri-State Health & Rehabilitation Center is considered to be one of the premier health care facilities in the Tri-State region. We attribute our wonderful reputation to one simple principle - our love and commitment to our residents. We continually seek new ways to serve our residents and enhance their quality of life. Whether the need is long term care or a short stay for rehabilitation, we believe it is essential to develop a personal and caring relationship with each resident. It is this "individualized" philosophy that sets us apart from other health care facilities throughout the region.
We are much more than just a nursing home. We are a home away from home for our residents providing outstanding skilled nursing care and a wide range of activities to meet the needs of a very diverse community of residents. Our rehabilitation department has received numerous awards and recognition throughout the area resulting in a successful return to the home setting for many of our rehab residents.
We welcome all inquiries and will provide tours for visitors each day during business hours. Please contact us today.
The activities department plays a critical role in helping our residents to maintain a fulfilling and active lifestyle. A wide range of in-house activities are provided each day along with off-site activities that are carefully structured and planned. Our residents maintain important social contacts and frequent outings provide continued interaction with the surrounding community.
Many of our favorite activities, which have been developed with the direct input and participation from our residents, are listed below :
Our Medicare and Medicaid billing team is constantly staying abreast of federal and state health care assistance regulations and they will gladly answer any questions you may have. You or your loved one may be able to receive financial assistance that you may not be aware of. Take for example... Just recently, a gentleman admitted his wife to our facility. He was concerned that his wife would not be covered financially and therefore unable to stay.
